The app shows my door as being open when it is closed and closed when it is open (same thing if I remotely open or close it....shows that it is doing the opposite of what it is doing...although the alerts I get on my phone when the door opens are accurate. Any ideas how to fix this?

Which MyQ product?  A MyQ garage door opener or a garage door opener using a MyQ Garage (a Wi-Fi Hub MyQ product)?

Test your photo eye safety system, that when the door is closing and you break the photo eye beam (see the opener manual for procedure), does the door reverse to the full open position?

I tested it and when I break the photo eye, it does not reverse and start to open again. However, the sensor appears to work because if the door is opening and I break the beam, the door reverses and starts to close. I guess it is flip-flopped the wrong way somehow.

From your test results, the opener's rail trolley is installed backwards. (Trolley is what travels back-and-forth on the rail that the chain or belt connects to and has a linkage bar to the garage door.)
If the opener was installed by a professional installer/dealer, then have the installer/dealer correct the issue at no cost. If a DIY installed, then the DIY person skipped the final testing steps to ensure the opener was installed correctly.
The rail will need to be removed, trolley slid off and reversed, and then reassembled.
The photo eyes are not working correctly, since the photo eyes are intended to detect an person/object in the path of a closing door and the photo eyes' person/object detection will force the door to reverse to the open direction, preventing injury to persons. For the safety of the household, the trolley should be corrected ASAP.

This DIY person had the same issue. Flipped the wire that goes from the motor to the motherboard and bam...back to the way it should be.
